Need a copy of your Continuing Education (CE) certificate from a CVMA event you attended? You can submit a request for your certificate using the form below.

This request form is available for attendees who participated in a CVMA webinar, seminar, or conference and need to obtain their CE certificate.

How It Works

  1. Complete the request form below with your information and details about the event you attended.
  2. Submit your request for your CE certificate.
  3. CVMA will review your request and follow up with you regarding your certificate.

 

Please provide as much information as possible, including the name and date of the event you attended, to help us locate your attendance records.
Once your request has been submitted, a member of the CVMA team will review your information and contact you regarding your CE certificate.

Please note: Certificate requests may require additional verification of attendance before a certificate can be issued.
